includes the following benefits:
A. Schedule of Benefits: Usual reasonable and customary charges, deductible and coinsurance apply.

B. Emergency Medical Evacuation and Repatriation:
In the event of a serious accident or illness, emergency treatment at a qualified medical facility is crucial. If qualified treatment is not available locally, the cost of an emergency evacuation can be from US$5,000 to US$100,000 - and require payment in advance. Patriot Travel Medical Insurance includes coverage for emergency medical evacuations to the nearest qualified medical facility or the country of citizenship; expenses for reasonable travel and accommodations resulting from the evacuation; and the cost of returning to either the country of citizenship or the country where the evacuation occurred up to the policy limit.
IMG® provides 24 hour a day, 7 day a week contact from anywhere in the world. Our on site medical professionals coordinate all aspects of an evacuation with the local medical providers, the transport company and the receiving hospital. Worldwide coverage ensures immediate access to qualified treatment for your medical emergency.

Patriot Travel Medical Insurance
covers:
- Expenses for Emergency Medical Evacuations to the nearest qualified medical facility or the country of citizenship up to the Policy Maximum.
- Expenses for reasonable travel and accommodations resulting from the evacuation.
- The cost of returning to either the country
of citizenship or the country where the evacuation occurred.
- Expenses for Repatriation of bodily remains or ashes to the country of citizenship up to a maximum of US$25,000 for a covered injury or illness that results in death.
C. Emergency Reunion:
During a medical emergency having a family member near by brings comfort and peace of mind. Patriot Travel Medical Insurance includes coverage for the travel and lodging expenses of a relative or friend during an Emergency Medical Evacuation. The travel expenses for the relative or friend may be either the cost of accompanying during the evacuation or traveling from the country of citizenship. The reunion expenses are covered up to US$15,000 for a maximum of 15 days. To make scheduling easier, all arrangements must be approved and coordinated by IMG.
D. Accidental Death and Dismemberment:
Patriot Travel Medical Insurance includes US$25,000 principal sum benefit for Accidental Death and Dismemberment. For covered Accidental Death or Dismemberment occurring during the period of coverage, Patriot Travel Medical Insurance will pay as follows:
- Accidental Loss of life: principal sum
- Accidental
Loss of two Members: principal sum
- Accidental
Loss of one Member: 50% of principal sum
"Member" means hand, foot or eye. For
additional information please see Conditions of Coverage section.
E. Special Coverages:
Patriot Travel
Medical Insurance sm includes the following special provisions:
- Incidental Home Country Coverage: During the period of coverage an insured person may return to their country of citizenship for incidental visits of up to two weeks total. The following conditions apply to this coverage:
- The insured person must have left their
country of citizenship.
- Total Period of Coverage must be for a
minimum of 30 days.
- The return to the country of citizenship
may not be taken to receive treatment for a condition incurred while traveling.
- End of Trip - Home Country Coverage: If you are enrolling in Patriot Travel Medical Insurance, for every six (6) months of coverage you purchase, you can purchase one (1) month of home country coverage up to a maximum of 2 months at the time of enrollment.
- Benefit Period - If a covered injury or illness has continuing treatment after the policy expires, the Benefit Period may provide continued coverage. When the policy expires, IMG will review the date of initial treatment for the covered injury or illness. If treatment began less than six months before the policy expired, benefits for the covered injury or illness will continue subject to the Policy Limits until there has been six months of coverage. The Benefit Period serves as an extension of benefits and does not limit the treatment time during the Policy Period.
F. Sports and Activities Coverage:
As outlined below, Patriot Travel Medical Insurance includes coverage for basic sports and optional riders for higher risk, named sports. The following is not covered:
- Amateur or professional sports or other athletic activity, which is organized and/or sanctioned, involving regular or scheduled practices, and/or regular or scheduled games. Unless a named hazardous activity, this exclusion does not include: Amateur athletic activities, which are non-contact and engaged in by an insured person solely for leisure, recreational, entertainment or fitness purposes.
- The following named hazardous activities are excluded but may be added by the Optional Sports Rider: Scuba diving, mountain climbing (where ropes or guides normally used), jet, snow and water skiing and snowboarding, sky diving, amateur racing, piloting an aircraft, bungee jumping and spelunking.
G. Eligibility Requirements:
The following conditions apply for all persons enrolling in Patriot Travel Medical Insurance
- For those over age 65 and visiting the US, your initial Period of Coverage must begin within 30 days of arrival in the U.S. Please attach a copy of your Visitors Visa to the Enrollment Form.
- This insurance is not available to non-US citizens who are residing in New York, California or Florida at the time of Application. This does not apply when the Effective Date coincides with the individual's departure date.
*If you are not in the US at the time of application, please indicate Expected Date of Arrival on Enrollment Form.
H. Common Carrier Accidental Death:
- If accidental death should occur while traveling on a commercial Common Carrier, US$50,000 will be paid to the beneficiary to a maximum of US$250,000 per family.
I. Additional Benefits for U.S. Citizens:
- Up to $500 will be paid for the sudden and unexpected recurrence of a pre-existing condition while traveling outside of the U.S.

This plan will pay up to $100 for the necessary treatment of sudden, unexpected pain to sound natural teeth.
